Referral Criteria and Clinical Management Guidelines

This page has been developed as a collaboration between the North East Wales NHS Trust and both Wrexham and Flintshire LHBs. It forms one part of the local health community's plans to more efficiently manage referral processes and ensure that patients receive care most appropriate to meet their needs.

The primary care guidelines for patients with suspected sleep apnoea are being developed jointly with North West Wales and North Wales Trust Central and are in draft and being piloted. We would welcome your comments and suggestions. Please contact lynne.morris2@new-tr.wales.nhs.uk

The snoring guidelines will be developed seperately. The draft primary care guidelines will be discussed at the next Regional meetings on 21st January and 9th February.

Falls Pathway

A Falls Workshop was held in July 2008 where the pathway was discussed and agreed with representatives from local Trusts, local health boards and voluntary organisations. The pathway has been jointly developed for service users to follow. Please refer to the Falls pathway and Falls Triangle.

Please note the Strategic Falls Board at North Wales East Trust have agreed the pathway will be implemented from January 2009.

General Surgery

Referral guidelines for Upper and Lower GI referrals as of the 1st of December 2008

Upper GI

Non urgent suspected cancer patients who are under 55 with reflux / dyspepsia should receive Helicobacter Pylori testing and at least a 1 month’s trial of acid suppression treatment before referral. Those patients who have not had the above will be referred back to the GP by the consultant with advice for further management in primary care.

Lower GI

All patients with rectal bleeding or a change in bowel habit, including Urgent Suspected Cancer patients, excluding those with a palpable abdominal or rectal mass, recent Fe deficiency anaemia or other valid reason should be referred to secondary care with the patient having completed a Patient Centred Questionnaire (PCQ). Where this is not the case the referral will be returned to the GP by the consultant with a request it be returned when accompanied by the completed PCQ. The patient will not be added to the waiting list for an outpatient appointment until the referral letter and the completed PCQ are received.
